Continuity of care is a patient-centric framework that addresses gaps in the diagnosis and treatment by providing an additional level of expertise, guidance, and monitoring. As part of the service, our doctors will be happy to join a virtual meeting or attend phone calls on short notice to discuss new findings or help you guide through the critical decision on treatment options.
